Social Prescribing Link Worker (Dover Town PCN)

Would you like to work for an innovative and progressive charity?  Involve Kent aims to improve health and empower people and communities to live well. We are looking for a Social Prescribing Link Worker to work with GP practices in Dover Town Primary Care Network (Buckland Medical Practice, The High Street Surgery, Peter Street Surgery, St James’ Surgery) to improve the wellbeing of their patients. Taking a holistic approach, you will connect people to community groups and statutory services to provide them with practical and emotional support.

You will need a positive ‘can-do’ attitude, high levels of resilience, excellent communication skills and the ability to motivate, empower, and support people to achieve their goals.

Please note, you must be able to drive and have access to a vehicle for this role. This role also includes providing regulated activity with adults therefore employment is subject to relevant checks with the Disclosure and Barring service.
