Children’s Health & Wellbeing Navigator

Working with GP practices to support children, young people and their families through diagnostic and support pathways for mental health, neurodevelopmental and other health conditions. Advocating and bringing together professionals to ensure a coordinated approach to their care and support. Helping to access information, local groups, and inclusive activities that they can enjoy.
We are looking for someone with excellent communication skills who enjoys working with children and families. You will have experience of working with a holistic and person-centred approach and strong IT skills with experience of entering personal data accurately. It’s important to be confident in a multi-disciplinary environment have a flexible, can-do approach and able to build positive relationships with other professionals as well as support the whole family.
You will need to be able to drive and have access to a vehicle for this role, as it involves community and home visits. The role includes providing regulated activity with children, so employment is subject to an Enhanced DBS check.
