Working with GP practices to identify and support children, young people and their families through a potential diagnosis pathway e.g. mental health, neurodevelopmental and other health conditions. Advocating for them and bringing together professionals as a Multi-Disciplinary Team to ensure a coordinated approach to their care and support. Helping them to access information, groups, and inclusive activities that they can enjoy.
