Leaving Care

 

If you’re a Care Leaver aged 18 to 25 or a Looked After Child aged 16 to 18 and preparing for your independence, you can access our Moving Forwards service.

Whatever your goal or dream may be, such as; starting your career, developing your talents, finding a hobby, gaining skills or dealing with worries or problems… We can help! 

Our Navigators understand the barriers and issues you might face and can help you achieve your goals.  

How we can help you

Working alongside Social Services Personal Assistants, we can support you for up to 12 months face to face, by telephone or virtually, prioritizing what matters to you, your goals, aspirations and helping you access opportunities, services, and activities to get your future on track. 

We can also help ensure you have a healthy future by making sure you have access to relevant medical and wellbeing services 

Our Care Leaver’s Service provides connection to education, employment, training and volunteering opportunities, helps you to access funds so that you can take part in activities and hobbies that interest you, and support you to build social contacts and knowledge for the transition to independence and adulthood.
