One-to-one Support
There are times where we all need some support, to be listened to, and given time to explore options that will help to improve our personal situation and our health and wellbeing. This may include aids to remain independent at home, access to benefits, or help to get out and meet new people.
We understand that it’s sometimes difficult to reach out and ask for help, so we will work alongside you at your own pace, and when you are ready.

How we can help
Working together, we will identify what matters to you, what is going well in your life, and areas you feel you may need some support with. Using the information they gather, our Navigators will draw up a plan of what your priorities are and what may be available for you to access, either through Involve, or via referral to other support services.
We provide:
- Support for adults struggling financially, using food banks, struggling mental health, have a long term health condition, or are socially isolated
- Practical advice and support for over 55’s to be supported to live safely and independently
- One to one support through your GP practice to access information and support in your community if you are feeling isolated, need support with housing, grants, benefits, or managing your health conditions
- Support following a stay in hospital
