What we do
Hospice at Home enables patients to receive specialist care and support provided by ellenor in familiar surroundings.
This includes an assessment of needs, specialist palliative care advice, symptom management, nursing care at the end of life, end of life care planning and crisis support.
When we have received a referral, the patient will be contacted to arrange an appointment for an assessment with the most appropriate member of the team. Depending on circumstances, an outpatient appointment may be offered or a home visit.
