What we do
Dementia Coordinators will be able to offer support and guidance pre, during and post diagnosis to people living in West Kent, North Kent and Medway. We support with many aspects of dementia and link in with GP surgeries and work collaboratively with others to ensure those living with dementia and their carers are supported and equipped to live as well as possible. Dementia Coordinators will be able to visit those who need support in their own homes.
