What we do
Providing care coordination, care navigation and social prescribing to children and their families where a child has a long-term health condition including mental health and neurodiversity.
Navigators are based in GP practices (see our website here for a list of locations) across Kent and Medway and support children and families to access support services, community resources and navigate diagnostic and therapeutic pathways.
