What we do
Providing lived experience support and advocacy to learning-disabled and autistic children and young adults (and their families) who are receiving complex care support from health, education, and social care via the Kent County Council Dynamic Support Service.
Navigating the complex care and support systems, providing practical and emotional support and connection to community services.
A holistic approach to reducing the risk of admission into Tier 4 provision or improve admission and discharge experience and outcomes across Kent and Medway.
