What we do
Our Hospital Social Prescribing service is for anyone aged 18 or over who is being discharged from Maidstone or Tunbridge Wells hospital who would benefit from some additional information, advice or support.
Our Hospital Social Prescribers will work with you to find our what is important to you to help to improve your wellbeing.
We can work with you on a one to one basis and provide practical support around housing, transport, benefit maximisation, social groups and more.
