What we do
The Hospice Community Team provides specialist care, advice and support to patients with a terminal illness in their own home environment. This includes a holistic assessment of needs, symptom management, sign-posting, planning for the future, urgent equipment support, medication advice, and end of life care support.
We work closely with your GP and other community/hospice services to ensure patients and their loved ones are receiving the care and support they need. Referrals are accepted from healthcare professionals, or by self-referral (we would confirm medical history with your GP).
Depending on circumstances, an outpatient appointment may be offered or a home visit.
